New York State's CARE Act

The New York State CARE (Caregiver Advise, Record and Enable) Act is a new law that requires hospitals to give patients the opportunity to name someone—a “caregiver”—who is willing to be part of planning for discharge and providing care at home. As long as the patient agrees in writing, the hospital can share medical information with the caregiver. The hospital is also required to instruct the caregiver about the kinds of care that will be needed at home. Similar versions of the CARE Act, which was developed by AARP, have been adopted in over 30 states.
