On November 10, 2008, the Community Health Foundation of Western and Central New York announced plans for its third collaborative to improve transitions in care. A key component of this new collaborative, expected to include health care providers serving one or more of the 16 counties in western and central New York State, will be the integration of the materials developed for Next Step in Care.
“We are pleased that health care providers are increasingly recognizing the value of the Next Step in Care guides and checklists, and their adoption as part of this important collaborative means they’ll find their way into the hands of more family caregivers and health care providers,” says Carol Levine, director of the Families and Health Care Project at United Hospital Fund, who also heads the Next Step in Care campaign.
